Disease Management and Counselling

At Yonge Cummer Pharmacy, our pharmacists help to address chronic conditions in their early stages and prevent the disease from progressing and developing any complications. There are a number of steps our pharmacists take in helping patients including designing and implementing care plans, completing assessments, and counseling around current medication regimens. Our integrated healthcare team, communicate and collaborate with each other to ensure chronic conditions are addressed early on to prevent progression.
